Scoliosis Screening
The 5@5 Rule — when to refer.
The Referral Rule
5@5
Five Degrees at Five Years
If a patient has 5° or more of trunk rotation on a scoliometer, at age 5 or older — refer to Hey Clinic. You don’t need to determine severity, curve type, or treatment path. That’s our job.
1
Adam’s Forward Bend Test — have the patient bend forward at the waist, arms hanging freely.
2
Place scoliometer at the apex of any visible trunk asymmetry or rib prominence.
3
5° or more = refer. No X-ray needed before referral. We’ll handle imaging at the first visit.
5@5 Tutorial Video

Scoliosis is missed at well-child visits more often than most physicians realize — not because of inattention, but because there’s no universally adopted screening protocol in primary care. The 5@5 Rule gives you a clear, evidence-based threshold that takes under 60 seconds.

You don’t need to determine if it’s AIS, congenital, or neuromuscular. You don’t need to order X-rays first. You don’t need to know the Cobb angle. If the scoliometer reads 5° or more in a patient age 5 or older — send them to us. We’ll do the rest and send you a complete report within 48 hours.

Hey Clinic sees patients from age 5 through adulthood. Early identification means more conservative options — and better outcomes for your patients.

Conditions We Treat
What we see — and what makes us the right referral.

We specialize in spinal deformity across the entire lifespan — from age 5 through adulthood. The conditions below represent our referral categories. If you’re unsure whether a patient is appropriate, call us. We’d rather talk through a borderline case than have you hesitate.

Adolescent Idiopathic Scoliosis (AIS)
Ages 10–18. Our core specialty. A new Hey Clinic study shows the rate of surgery can be reduced 65%* for growing children with Schroth exercises and Rigo 3D bracing. Refer at Cobb ≥20° or trunk rotation ≥5°.
Adult Scoliosis & Spinal Deformity
De novo adult scoliosis, progressive adult deformity, pain with deformity. Conservative management and surgical correction. Refer patients with back pain + visible deformity or imaging-confirmed curve.
Kyphosis & Scheuermann’s Disease
Postural and structural kyphosis in adolescents and adults. Schroth has specific kyphosis protocols. Scheuermann’s kyphosis requires distinct approach from AIS — subspecialty evaluation indicated.
Congenital Scoliosis
Vertebral anomalies present at birth. Requires early identification and monitoring — different management from idiopathic. Refer any child with suspected vertebral anomaly on imaging.
Neuromuscular Scoliosis
CP, muscular dystrophy, spina bifida, Chiari malformation, Marfan syndrome. Surgical cases requiring subspecialty deformity expertise. Refer early — progressive curves are common.
Early-Onset Scoliosis (EOS)
Scoliosis before age 10. Growth-friendly surgical strategies. Serial casting and bracing options. Refer any child under 10 with suspected scoliosis — early intervention is critical.
Revision Spine Surgery
One of our most requested referral categories. Flatback deformity, pseudarthrosis, adjacent segment disease, proximal junctional kyphosis. 4,500+ deformity corrections — revision expertise.
Spinal Stenosis (Adult)
Lumbar and cervical stenosis. Conservative and surgical management. Refer patients with radicular symptoms, claudication, or imaging-confirmed stenosis who are not responding to conservative management.
Spondylolisthesis
Adolescent isthmic and adult degenerative spondylolisthesis. Graded management from activity modification through fusion. Refer patients with persistent symptoms or slip grade II+.
